Before you begin an application, please carefully read the eligibility requirements at: www.coloradohealthservicecorps.org  Program eligibility is determined by federal and state statues, which must be applied without exception. Also before you begin, it may be helpful to have the following documents at the ready:
 

• current resume or curriculum vita
• most recent loan statement from each of your lenders
• two letters of support (one from employer if employed; one from faculty supervisor if in residency)

Retain your user name and password until you have completed your application. You will be prompted to verify your user name and password again after you leave this page.
